Care By Design Long Term-Care Conference

The Care by Design Conference is an opportunity to connect with colleagues with an interest long term care and learn about new standards of care, clinical practice guidelines, and emerging industry trends. The conference aims to provide an enriching and engaging interdisciplinary education opportunity annually. 

By the end of the conference, the learner will:

1. Review current evidence and best practices in dementia syndromes, hospital admissions, and heart failure for long-term care (Medical Expert, Scholar).
2. Apply practical tools and strategies to improve resident care and quality of life (Medical Expert, Health Advocate).
3. Enhance interdisciplinary collaboration and communication with long-term care team members (Collaborator, Communicator).
4. Identify opportunities for quality improvement and leadership within long-term care settings (Leader).
5. Reflect on professional and ethical responsibilities in the care of long-term care residents (Professional).
