This 1.5-day live virtual workshop teaches health care professionals:

Day 1: How to investigate patient safety incidents in health care with a system-level approach (incorporates human factors engineering principles) using root cause analysis.
Day 2: How to identify potential failures in a system and redesign processes to mitigate risk using failure mode and effects analysis.

Target Audience: Physicians, nurses, pharmacists, pharmacy technicians, pharmacy managers/directors, risk managers, patient safety officers, medication safety officers, and other health care practitioners

Requirements: Computer with microphone, webcam, speakers, and internet connection; lined paper and pen; wall space to use for hands-on-practice
