Presented by NSH Primary Health Care in partnership with IWK Health, , the goal of this session is to explore what advance care planning is and what to include when creating a personal directive.

Are you interested in preparing you and your loved ones for the future? Do you have life insurance? A will or a power of attorney? There is another kind of insurance you can have, that costs nothing, and helps ensure that your medical and personal care wishes are respected.
